An inevitable accident is an occurrence that could not have been avoided or foreseen despite the exercise of ordinary care, caution, and skill by the defendant. If harm results from such an accident, rather than from any negligence on the defendant's part, the defendant is not liable.

In Stanley v. Powell (1891), a member of a shooting party fired at a pheasant; the shot deflected off a tree at an angle no one could have anticipated and struck another member of the party. The court held this was an inevitable accident, since ordinary reasonable care had been taken and the deflection could not have been foreseen.

Don't treat "the defendant did not actually foresee the harm" as enough to establish this defence. The test is objective: the defendant must show that a reasonably prudent person, taking every precaution a reasonable person would take, could not have foreseen or avoided the harm either — not merely that this particular defendant happened not to.
